Abstract

Papillary Thyroid Cancer (PTC) in both, Thyroglossal Duct Cyst (TGDC) and Ectopic Thyroid (ET) are very rare. We report a 37 years old man with this association.

Introduction:

PTC in TGDC is 1% of all thyroid cancer, upon 30 years old. PTC in ET is less than 1% of all thyroid cancer. PTC in TGDC and ET in the same patient is extremely rare, maybe less than 0.5%

 

Clinical Case

Male, 37 years old who four years ago had a cervical midline tumor in front of hyoid bone associated to dysphagia and progressive increase in the last year.

US and CT showed a 7cms. tumor with normal thyroid gland without nodes. Function thyroid gland was normal and fine needle aspiration was lymphoid reaction.

We made Sistrunk’s operation and pathology report was a 5cms. moderately differentiated PCT in TGDC and ET with the neoplasia, without lymphovascular invasion.

Two months later we made total thyroidectomy and pathology informed normal thyroid. Today patient is well and taking hormone replacement with levothyroxine.

 

Discussion

It’s a very interesting case report by the casual find of PTC in TGDC and ET after Sistrunk’s operation, extremely rare association in one patient and the case management.

We made total thyroidectomy by gender, age, size, diffentiation grade and multifocality.

Persistence of thyroid tissue in TGDC explains the presence of PTC inside the TGDC with ET.
