Reconfirmation of the Usefulness of Taller Than Wide Sign by Multi-center Collaborative Research in Japan

It was described as suspicious findings of malignancy in thyroid nodules that an anteroposterior dimension-to-transverse dimension ratio (AP/T ratio) is larger than 1, which is called 'Taller than wide sign'. We reconfirmed the usefulness of the AP/T ratio by multicenter collaborative research.



At registered 11 institutes in Japan, 2032 nodules were detected by ultrasonography and diagnosed pathologically after surgery. The nodules consisted of 896 of benign tumors and 1136 of malignant tumors (994 of papillary carcinomas, 101 of follicular carcinomas, 41 of others).



The AP/T ratio of malignant tumors was significantly larger than that of benign tumors. The AP/T ratio of papillary carcinoma was significantly larger than that of benign tumors, but that of follicular carcinoma was not different from benign tumors.



In this study, the result showed that the AP/T ratio was useful for the diagnosis of malignant tumors entirely, as already reported. But most of the malignant tumors were papillary carcinoma. The AP/T ratio of follicular carcinoma did not show any difference from benign tumors. For the usefulness of AP/T ratio of malignant tumor other than papillary carcinoma, the further investigation seemed required. When we investigated in advance how much observer-variation occur depending on the orientation of the probe and the magnitude of the tilt for multicenter collaborative research, the data in the horizontal cross section was the least variable.
